Shore Home Care is a locally-owned, hospital-based home healthcare company. We are a member of Shore Health System, which includes Dorchester General Hospital in Cambridge, Maryland and The Memorial Hospital at Easton, Maryland. Hospitalization can be an emotional and financial burden for any family. However, with the help of advanced medical technology, many illnesses that once required hospitalization can now be treated at home. Experience has shown that patients recover more quickly when surrounded by loved ones in the familiar settings of home. Home care has proven to be more economical than hospitalization. Shore Home Care is the only organization in this area which provides a full range of home healthcare services.

Service Area

Nursing, rehabilitation therapy, home health aides, social work, and Healthwatch personal response program services are available throughout the mid-Shore region of the Delmarva Peninsula. Hospice services are available in Talbot and Caroline counties.

Patient Education

Patient and family education are an integral part of the home care plan of care. Patients are better able to improve their health status if they are an active participant in their care. A comprehensive assessment is performed on admission to identify patient needs for teaching, observations and safety. Education takes many forms. It may be a nurse teaching the family of a hospice patient how to provide quality of life and comfort to their loved one, or a physical therapist working one-on-one with a patient to improve ambulation safety in the home environment, or a nurse teaching a patient/family wound care techniques.

Coordination of Services

Our multidisciplinary team delivers integrated home care services, working closely with the patient, family and the physician to provide the most appropriate care. Discharge planning begins with the first patient contact, which can often occur prior to hospital discharge. Staff provide a smooth transition from hospital to home, and continue education activities until the patient is able to manage home care independently.

Access to Care

One phone call to our central intake department provides access to all of our services. Our knowledgeable customer service representatives will help you with all your questions. Medical professionals making a referral will need to provide Shore Home Care with the patient's name, address and phone number as well as your name and phone number. Let us know what services the patient requires, and what your expectations or goals are.

Insurance Coverage

Home care services are covered by most insurance payors. As a service to our valued patients, Shore Home Care processes billing on behalf of the patient to most major insurance companies. Our billing department representative will work closely with you, your physician and your insurance company. Our staff are also familiar with payor requirements for coverage, such as "homebound" necessity, criteria for coverage of medical equipment, or home infusion therapy. "Homebound" is defined as having difficulty leaving the home without significant assistance. Patients who occasionally leave home for short periods of time, or who can leave home to receive medical care, can still be considered "homebound". The homebound requirement is a criteria for eligibility for some major insurance payors, including Medicare and Medicaid.
