Taking a Comprehensive Rehabilitation Approach
Shore Health System provides a Comprehensive Rehabilitation Program on both an Inpatient and Outpatient basis. This comprehensive rehabilitation "continuum of care" includes traditional and complementary interdisciplinary services. Comprehensive Inpatient Rehabilitation is provided at the Requard Center for Acute Rehabilitation, a 20 bed licensed rehabilitation hospital unit inside the Memorial Hospital of Easton.. It provides intensive rehabilitation programs, daily rehabilitation physician and 24/7 rehabilitation nursing services to inpatients with significant physical and/or cognitive functional limitations resulting from illness or injury.
The Requard Center serves as the "core" of a comprehensive rehabilitation network of care which also includes physical rehabilitation services within other areas of Shore Health System's two acute care hospitals (Memorial Hospital of Easton; Dorchester General Hospital), and at it's Outpatient Rehab Centers in Easton, Cambridge, Centreville and Denton, the Balance Center, The Neuro Day Rehabilitation Program, the Physical Therapy (PT) and Sports Medicine Center at The Orthopedic Center in Easton, and the Center for Integrative Medicine.
Shore Rehabilitation Services also participates as a member of the University of Maryland Medical System's Regional Rehabilitation Network. Our Easton outpatient rehabilitation center and UMMS's Kernan Hospital are the only two facilities in Maryland that provide world class robotic gait rehabilitation using the Lokomat gait system.
Outpatient rehabilitation services also focus on a comprehensive multi- inter disciplinary approach The needs of many persons served by Shore Rehabilitation Services require more than a single service such as physical therapy (PT). Many patients or clients benefit from a team that may include rehabilitation physicians, rehabilitation nurses, case management and very likely Occupational Therapy and/or Speech therapy in addition to Physical Therapy. For example a Stroke patient discharged home may still have significant difficulty with their ability to speak and also daily activities such as dressing or bathing in addition to their difficulty with strength, standing and walking. We provide a means that all these conditions can be treated at one place by a coordinated team of professionals under the guidance of our rehabilitation physician medical director. This comprehensive approach extends across all ours services and locations to provide the right team when the need exists.
Programs also include complementary and alternative healthcare services including acupuncture, massage therapy and others services provided by the Center for Integrative Medicine.
Shore Rehabilitation Services operates in an "integrated" and collaborative manner with the other healthcare programs/services in the Shore Health System continuum of care including but not limited to Shore Home Care, the Regional Cancer Center, the Pain Management Center and the Joint Replacement Center.
The primary market it serves includes Talbot, Dorchester, Caroline, Kent and Queen Anne's counties. The secondary market includes surrounding counties in Maryland and Delaware. Tertiary markets include major metropolitan hospitals in Baltimore, Washington DC and other cities where patients may travel for specialty healthcare services, and return home for rehabilitation. The Rehabilitation Program shares the mission of Shore Health System, which is to "Excel in Quality Care and Patient Satisfaction" through "Exceptional care, Every day".
Populations Served
Shore Health System's Shore Rehabilitation Services is focused on optimizing functional independence and highest quality of life at home, at work, at school or at play. Patients are referred to the inpatient and outpatient programs after acute hospitalization, from home, emergency rooms, inpatient rehabilitation facilities, nursing homes and outpatient healthcare centers including physician offices. Persons served include a wide variety of persons with significant impairments, limitations, and activity restrictions due to illnesses, injuries and occasionally congenital conditions. The following is a list of typical conditions for which patients would be admitted to the Program. It is a representation, and not inclusive of all possible patient conditions requiring rehabilitation:

Outcomes Management
Outcomes management is focused on accomplishment of predetermined goals or objectives in several specific key areas in a person's plan of care. Primary focus and first priority is given to meeting the goals of the individual patient, and successfully achieving these clinical outcomes in the most effective and efficient manner possible. Clinical Outcomes as well as other key program indicators are measured, reported, and acted on. All indicators serve to support and optimize performance in meeting all the expectations of the program. Our program evaluation utilizes a combination of external and internal benchmarks in order for the program to compare itself to national or regional performance. Inpatient rehabilitation at the Requard Center uses the FIM (Functional Independence Measure system) and outpatient centers use the FOTO system. Both of these systems are accepted by Medicare as methods to track the effectiveness and the cost efficiency of the rehabilitation programs. Our use of these systems affords the patient and their family additional confidence in the quality and cost effectiveness of our services.
